Gluten is an amorphous protein found in the seeds of many cereals (wheat, barley, rye, buckwheat, kamut, triticale and possibly oats) combined with starch. Represents 80% of wheat proteins and is composed of gliadin and glutenin. Gluten is responsible for the elasticity of the dough and gives the elastic and spongy consistency of baked breads and pastries.

Gluten can be obtained from the flour of wheat and other cereals, washing the starch. The resulting product will have a sticky, fibrous texture similar to chewing gum. For this reason food is prized for its thickening power.


WHAT IS CELIAC DISEASE?



Celiac disease (CD) is a permanent intolerance to gluten in wheat, barley, rye and possibly oats that occurs in genetically predisposed individuals, characterized by an inflammatory reaction, immune based in the mucosa of the small intestine impairs absorption of macro and micronutrients.

The estimated prevalence in Europeans and their descendants is 1%, being more common in women with a 2:1 ratio.

A significant percentage of patients (75%) are undiagnosed because, for the most part to the EC for years has been associated exclusively with its classical clinical presentation. However, the recognition of atypical forms of manifestation, oligo-and asymptomatic, combined with the highest and best use of available laboratory tests, has allowed to reveal the existence of different types of EC:


• Symptomatic: The symptoms are very diverse but all patients showed serology, histology, and genetic testing compatible with the EC.
• Subclinical: In this case there will be no symptoms or signs, although they will be positive the rest of the diagnostic tests.
• Latent: Are patients at any given time, consuming gluten, have no symptoms and normal intestinal mucosa. There are two variants:

or Type A: EC were diagnosed in childhood and recovered completely after starting a gluten free diet, remaining subclinical state with normal diet.
or Type B: In this case, due to a previous study found that the intestinal mucosa was normal, but later developed the disease.

• Potential: These people have never submitted a biopsy compatible with the EC but, just as previous groups, have a genetic predisposition determined by the positivity of HLA-DQ2/DQ8. Although serology can be negative, have an increased number of intraepithelial lymphocytes. The average chance of developing active CD is 13% and 50% latent CD.
